General Questions
Is the Peace of Mind Registry the same as the registry for organ donation?
No. They are completely separate registries, though both are administered by Gift of Life Michigan. The Michigan Department of Health and Human Services oversees the Peace of Mind Registry.
What type of document can I register in the Peace of Mind Registry?
You can record a durable power of attorney for health care or an advance directive for mental health care. For more information on these and other types of advance directives, click on "Resources."
Can I register my last will and testament?
No. The registry is limited to documents pertaining to your future health care.
Can I register my advance directive using the U.S. mail rather than a computer?
Yes. You can mail the document to Gift of Life Michigan, at the address listed under "Contact Us." Include a separate piece of paper with the following information (please print): your name, address, telephone number, e-mail address (if you have one), and the last four numbers of your Social Security number.

We will send you back the original document and a wallet card.
Once my advance directive is registered, should I still keep the original document and copies of it?
Yes.
Will anyone at Gift of Life Michigan review my advance directive to ensure it meets requirements to be legally binding?
No.
Can I give access to the Registry to my patient advocate and to all family members?
Yes. You can share your password with whomever you choose.
If I register my advance directive, can I still change my mind?
Yes. You always have the right to close your account. Or you can record a new advance directive to replace an advance directive you filed in the past.
When will my doctor and other health care providers have access to my advance directive?
At this time Michigan health care providers do not have electronic access to your advance directive. You may present your wallet card to a health care provider so they can request a copy of your advance directive.

Technical Questions
What information is required to create an account?
You will need to provide your first and last name, current address, sex, date of birth, email address, and the last four digits of your social security number.
My husband and I use the same email address. Do we need separate addresses for the registry?
No, your accounts can have the same email address with different login and contact information.
What if I forgot my password?
What if I no longer have access to the email address used to create my account?
Click here to contact us and have that information changed. You will be required to verify the information you used to create your account.
What file types can I upload?
Any standard document formats (PDF, Word, etc.)
Can I delete an old directive?
No, but you can replace it with a new directive. If you no longer want your old directives to be stored, you can delete your account and all associated directives. Click here to contact us to request your account be deleted.
